Promote aging in place by coordinating available community supportive services for residents in low-income housing facility.

Duties include but are not limited to:

1. Maintains a network of service providers
2. Handles general case management
3. Maintains good relationships with residents and staff
4. Collaborates with the facility management staff
5. Assesses residents who are frail or at risk
6. documents resident incidents
7. Provides health/wellness education
8. Maintains visibility within the community
9. Other duties as assigned
